DQ Files/Maintenance/Log Audits

1. Preview Driver’s Qualification File for completeness.

2. Verify past employment (new hires less than 2 weeks working for the transportation
company).

 *2 years drug and alcohol follow-up
 *3 year past employment follow-up
 *MVR verification (if not provided by the new hire).
 *Review Medical Long Form for completeness and compliance.

3. Scan all the required Driver’s File forms into a computerized program.

4. Make a back-up copy of all of the Driver’s Qualification Files.

5. Have all the Driver’s Qualification Files available at the time of a State or US DOT
Compliance Review.

6. Send the transportation company a warning notice before a driver’s license expires or
medical certificate  expires, or annual review of a particular driver is due.

7. If the transportation company is using the log check program, the logs will be
checked on a monthly bases for the following:

 *Form and content
 *Accuracy / true and correct
 *Compliance with FMCSR 395 HOS Rules
 *A re-cap listing any violations
 *The transportation company is responsible for discipline for logging violations within
their company.
 *Medical Form Review

8. The transportation company is responsible and must arrange with the independent
contractor to get them the driver’s logs for each month.
